Preventing homelessness when leaving hospital
You should tell hospital nursing staff as soon as possible if you think you might be homeless after leaving hospital.
Hospital discharge team
They will ask the hospital discharge team to help. If you consent, the discharge team can make a referral on your behalf to any local council you choose. If you apply to us, you will be asked to complete a housing options assessment form (online form) which will be assessed. Please ask your discharge team to assist in completing this, where possible.
Adapting your home
Your stay in hospital might mean you need to change the way you live when you come out. For example, if you will face difficulty getting around or mobility problems. In this case, you may want to consider adapting your home, so you don’t have move out.
Adaptations might include things like, ramps, stairlifts, grab-rails, hoists, walk-in showers etc. You can find out more about how we can help from the home improvement agency.
Disabled Facilities Grants
These are paid by the council to help towards the cost of adapting your home. You can apply whether you own or rent your home. However, if you live in a private rented property, you must get your landlord’s permission. Your landlord can also apply on your behalf.
Apply if you:
- Are registered or eligible to register as disabled and
- Continue to live in the property as your main residence
