What is my Pathway Plan?

Your Pathway Plan maps out how you, your personal advisor, and the other services that support you are going to help you achieve successful independence. It’s a bit like a joint agreement between you and Future First, which you both have to agree on.

It’s important that you lead your Pathway Plan, and that it is your plan with your voice. You should use your plan to explain your needs, views, and future goals, and in return your personal advisor and their manager should use it to explain exactly what support they will provide you with in return.

Reviewing your plan

Your plan is reviewed every six months or whenever something important happens in your life. You and your personal advisor will review the targets you’ve set yourself for your journey to successful independence and decide on next steps. Your PA then supports you, over the next six months to achieve the goals you have set for yourself. You should always sign your plan after it’s agreed and your PA should always give you a copy to keep.

In your plan, your personal advisor and Future First will consider with you what extra support you may need because of your own individual circumstances.
