Wandsworth Health and Care Plan

The Wandsworth Health and Care Plan 2022-2024 sets out areas where, over the next two years, we can have the greatest impact by working collectively. The Health and Care Plan has been developed in partnership with health and care providers, the voluntary sector and residents.

We want people to remain as healthy as they can for as long as they can. We are focused on prevention, joining up care where it is appropriate.

Better Care Fund

The Better Care Fund (BCF) is one of the government’s national vehicles for driving health and social care integration. It requires Integrated Care Board (ICB) and local authority to agree a joint plan, owned by the Health and Wellbeing Board (HWB). These are joint plans for using pooled budgets to support integration, governed by an agreement under section 75 of the NHS Act (2006).
