Community wards

DescriptionThere are four 'Community Wards' in Wandsworth which provide support to people with long-term health conditions in their own homes, with the aim of helping to avoid emergency hospital visits and admissions.

The service takes a highly responsive multidisciplinary approach and is available to patients who have chronic, long-term health conditions and are registered with a Wandsworth GP. The wards also supplement existing health, social and GP services to support the earlier discharge of patients back home wherever possible.

If you have a long term health condition your GP, consultant or another health care professional may refer you to a community ward for further support.

- North Wandsworth (Battersea)
- West Wandsworth (Roehampton/Putney)
- South Wandsworth (Balham, Tooting & Furzedown)


The community wards bring together the following services:

- GPs
- Community matrons, specialist nurses and district nurses
- Palliative care
- Community physiotherapy
- Occupational therapy
- Community pharmacy services
- Intermediate care team
- Mental health services, including drug and alcohol services
- Social services
- London Ambulance Service
