DescriptionTrinity Hospice's Outpatient Service provides a range of care and support to patients and their carers or family members. The service offers different sessions depending on the person’s needs, including group sessions and individual sessions.

The services available are:
- Arts therapy
- Clinics with a Clinical Nurse Specialist
- Complementary therapies
- Consultant medical clinics
- Occupational therapy
- Massage, aromatherapy, reflexology and Reiki
- Physiotherapy
- Psychological support
- Social welfare advice and
- Spiritual care

When someone first visits, an individual care plan is made for them and this is then reviewed throughout the time they receive support from the service. If someone’s symptoms have stabilised and they have no other pressing needs, they will be discharged from the outpatient service, but they can be re-referred again if needed. Patients must be mobile and able to travel to Trinity Hospice to access this service.

EligibilityIf you would like to be referred to Trinity Hospice, please ask your health care professional to refer you. Healthcare professionals, such as a doctor or nurse, may refer anyone aged 18 and over with a progressive, incurable disease who have a prognosis of less than two years. This can be cancer or other non-malignant diseases.

People may be referred when the complexity of their illness needs the services of a specialist palliative care team. They must have a therapeutic need and can be referred for symptom control, psychological or spiritual support. If you would like some guidance or more information about referring someone, please contact the team. The service understands the complex needs of palliative patients and considers each referral on an individual basis.
