Do you need help to manage at home
Although most people leaving hospital will manage at home without any extra care or support, going home from hospital can be an anxious time. You may be worried about how you will cope, or about how to organise the care and support you will need when you get home.
We work closely with hospital staff to make sure your discharge is well planned and that you have the care and support you need when you get home. We aim to help you to be as independent as possible.
If you think that you may have difficulties returning home, talk to your named nurse or the discharge co-ordinator on the ward and they will refer you to the social worker.
If you find you cannot cope at home
We hope you will manage well once you get home but sometimes, even with careful planning people find that they cannot cope as well as expected.
If you have difficulties coping once you are home you should tell your GP or district nurse or contact the access team on 020 8871 7707.
