Supplementary information forms
Primary schools which require a supplementary information form are:
- All faith schools (Church of England, Catholic and Muslim)
- Hotham School (bilingual class only)
- Wix School (bilingual class only)
The faith school supplementary forms are available direct from the schools and must be returned to the schools by the closing date of 15 January 2012.
If you are applying for a place in the bilingual class at Hotham School or Wix School, you must obtain the form from and return it to Pupil Services (details below) by the closing date of 15 January 2012.
