It means changing the way people receive care and support from Adult Social Services.

In 2007, the government launched Putting People First, a shared vision and commitment to transform adult social care.

The aim is to personalise adult social care. This means putting people at the centre of the decision-making about their care and support.

It means enabling them to have choice and control over the care and support they receive.

It means supporting people to not only meet their needs, but to achieve their goals and be equal and active citizens.

Transformation is not about changing our values - it's about changing our practice to better support our values.

Transformation is about spending the Council's money as efficiently as possible to provide effective care and support.

What is transforming in Wandsworth? 

There are five areas that we are focusing on. They are all explained further over the next few pages.

  1. We are working in partnership with people who are supported by Adult Social Services, their carers and other local citizens to shape the transformation.
  2. We have changed the way we support adults with eligible care and support needs. We have introduced self-directed support and eligible adults will now receive a Personal Budget.
  3. We are developing our preventative support. This means offering people some early support to help them keep or get back their independence as quickly possible. This prevents people needing a lot of support later on.
  4. We are strengthening our information and advice service so that all residents can access information and advice about different care and support needs, the options available to meet these needs and how to pursue these options.
  5. We are developing a more varied and flexible care and support market. The 'market' is all of the things that residents might want to buy to meet their care and support needs. This includes services like home care and respite, but care and support needs are about a more than personal care. They're also about things like being part of the community, having access to employment opportunities and keeping safe. We are developing the market to make sure there are provisions available to meet a wide range of needs.
