| Description | The Wandsworth Community Learning Disability Team (CLDT) is a multi-disciplinary team comprising of professionals from both Social Services and the NHS. The aim of the team is to provide advice and support to people with learning disabilities and their carers/family. The CLDT works with adults 18 years of age and over, who have a learning disability. People with learning disabilities are people and citizens first, and are entitled to access the same health and social care services that everyone else does. However, sometimes people with learning disabilities may, for a period of time, require some specialist advice and support, which is where the CLDT can help. Wherever possible, support from members of the CLDT is 'time-limited', however you can always contact the CLDT again if you need more help or advice at a later date. |
