Social services - Hospitalisation - Help returning home

You may need help when you leave hospital to continue to live in your home.

We can help people who need some assistance to get back on their feet while they recover fully, or to provide the help to those people whose circumstances and needs change following an accident or illness, such as a stroke.

You will have a home assessment visit with an Occupational Therapist to check you can access all areas of your home and determine if you need specialist equipment or adaptations to make your life easier.

Eligibility

Referrals are made by NHS ward staff and the council's care managers as part of a community care assessment.
